PHOTO and/or VIDEO RELEASE I,

$_______________, agree to allow Raymond J. Lobell Jr. (dba
MostMuscular.Com, MostMuscular.Com ULTRA, and
MostMuscular.Com ULTRA Plus), to publish on and use to
promote the aforementioned websites and our branded YouTube
channel, tobydog22, photos (at a maximum resolution of
approximately 1000 by 600 pixels) and/or video footage of me
taken on _____________ in
______________________________. PHOTOS: I
understand that I will receive a CD or DVD containing the
digital files from the photo shoot, and that I am licensed
to reproduce, sell, and/or distribute these images in any
way I see fit. VIDEO: I also
understand that I understand that any video footage of me
(or still images derived from that footage) will be used on
the aforementioned websites, and may be used as part of a
composite DVD production that will be produced, edited,
copyrighted and sold by MostMuscular.Com, and that
MostMuscular.Com will be exclusively entitled to revenues
from the sale of these DVD productions. I understand that I
may receive at no charge three copies of any DVD in which I
appear if I request them via email, telephone call or in
writing after the DVD is released, and that I may purchase
additional copies at $5 each. I further
understand that I am an independent contractor for the
purposes of the shoot(s) described above, and that I will
hold harmless Raymond J. Lobell, Jr. and the aforementioned
websites for any injuries sustained during the shoot(s)
